The OF-306 is used to determine your acceptability for Federal employment and your enrollment status in the Government's Life Insurance program.
The OF-306 is a form submitted in addition to your primary application document. It is not your primary application document (although the information in both the OF-306 and OF-610, a primary application document, is contained in the single SF 171, also a primary application document). You may be asked to complete this form at any time during the hiring process. Follow the instructions that the agency provides. If you are selected, you will be asked to update your responses on this form and on other materials submitted during the application process. You will then be asked to re-certify that your answers are true before you are appointed. As with all application forms, it is strongly recommended that you type the information.
